Office Hours
Our office hours are Monday through Friday from 8:45 am to Noon, and from 1:00 pm to 4:45 pm. Office hours are by appointment only.
Preparing for the Office Visit
We ask patients new to our practice to arrive 15 minutes prior to their scheduled appointment to allow time for the registration process. You will need to bring:
- Insurance Cards
- Photo Identification
- Current Medication List
- Co-Pay/Co-Insurance
- New Patients: If you are insured by an HMO, please change Primary Care Physcian prior to your office visit.
Methods of Payment
We accept Cash, Checks, Mastercard, Visa and Discover.
Cancellation Policy
If for any reason you cannot keep a schedule appointment, please phone to cancel or reschedule at least 48 hours in prior to your scheduled appointment time. This will allow us to better serve all of our patients.
Insurance
Please call our office for insurances accepted by our practice.
Prescription Refills
To make your office visit more effective, please inform your physician of any prescription refills needed at the time of your appoinment. Please include
- The name of the drug
- The Dosage
- The frequency at which you take the drug
- The name and telephone of your pharmacy
Sick Visits
If you are having a medical problem between routine office visits, please call and schedule a sick visit. Telephone calls are answered in the office between 8:45 am and 4:45 pm Monday through Friday. If you call the office during these times and the call is answered by the voice mail Do Not Hang Up, this simply means that the telephone staff are busy with other calls. Please leave a message stating your name, date of birth, telephone number and the reason for your call. Voicemail messages are answered throughout the day, and call-backs are made by appropriate personnel, when indicated.
Referral Policies
If your insurance requires a referral to see a specialist, please allow 3 business days to process the referral. Medical forms that must be completed by the physician will necessitate an office visit.
Lab Results
By providing us a self-addressed envelope, we will mail your lab results directly to you.
Disclosure of Medical Information
Due to HIPPA, a federal privacy policy law which we must abide by, we are unable to disclose your medical information to anyone without express permission from you on a Release of Information Form. There may be a charge associated with copying your medical records. If you have a Healthcare Proxy, be sure to provide us with a copy for your file.
